Enclosed you will find your puppy’s registration papers. Please take a moment now to look at them carefully. As per the ISSR, Inc. rules this application will become VOID once your puppy reaches 210 days of age unless an extension has been submitted, so please processes them ASAP!!

Before submitting your papers to the ISSR please take the time to re-read the instructions on the application.

1) You will need to select your puppy’s future name. Since “ZION” is a registered/licensed kennel, you must use it as a prefix or suffix. If you fail to do so the ISSR will automatically prefix your puppy’s chosen name with “ZION.”

Example: If you write HAMILTONS GIANT BABY BIGFOOT into the squares (this will take up 29 blocks) the ISSR will automatically prefix your selection with the “ZION” prefix and your puppy’s name (for life) will be ZION’S HAMILTONS GIANT BABY BI. The “gfoot” will automatically fall off due to the fact that you only get 30 blocks/spaces!

Please Note: If you are planning to breed, now is the time to select your kennel name and register it so that both kennel names can appear on your puppy papers!!

2) You will need to enclose 2 pictures of your puppy.  Please see instructions on top left hand corner of your puppy's papers. These are for identification only, but your application will be refused if they are not enclosed!!

3) Please be sure to enclose the $25 processing fee and mail to the address listed at the bottom of your registration application.

Please Note: The ISSR, Inc. will ONLY accept 2 owners. You cannot submit papers that list “John & Mary.” Each INDIVIDUAL must be listed separately. If this puppy is a pet and will be owned by “John & Mary” Smith, then in the “NAME” blocks above "the parties this dog was transferred to" section will state JOHN & MARY SMITH (on line 1). In section B under owner, “JOHN SMITH” must sign and under “CO-OWNER” MARY SMITH must sign. Please check the instructions carefully to avoid having your papers returned!
